If you are a new patient, information is being sent to you in an effort to streamline the check-in process and answer any questions you may have regarding your visit. We will be contacting you a few days prior to your appointment to confirm the appointment date and time.

Billing

West Avenue Family Dentistry participates with many major and smaller insurance plans. If we do not participate with your insurance and your plan allows out of network benefits, our office staff will assist you with the necessary paperwork so you can receive reimbursement directly from your insurance company.

Appointments & Cancellations 

If you need to reschedule an appointment we require 24 hours notice. In such a case, please call our office and arrange for a make-up appointment with one of our patient care coordinators.
